APPOINTMENT ENQUIRY

Tell us how to reach you.

Complete the short form below. Preferred dates and doctors are not guaranteed; the hospital must confirm them. Required fields are marked *.

Dates, times and doctor choices are preferences, not available slots. The hospital will confirm the final appointment.

Please do not include test results, diagnoses or sensitive medical information in an initial enquiry. Do not use this form for emergencies.

HOW IT WORKS

Four steps to request your visit

There are no automatically confirmed slots on this page. You'll need confirmation from the hospital team before travelling.

01

Choose your department

Select IVF & Fertility or Kidney Care and, if you wish, your preferred doctor.

02

Share your preferred visit time

Select a date and time for your enquiry. These are preferences, not live appointment slots.

03

Send your enquiry

The form tries the hospital's appointment intake. If it is unavailable, use the clearly marked contact alternatives.

04

Receive hospital confirmation

Only the hospital can confirm the doctor, available time, fees and final appointment.

PATIENT QUESTIONS

Appointment enquiries: common questions

Useful answers before sending your request. Contact the hospital to confirm consultation timings and fees.

Does selecting a date reserve an appointment?

No. The date and time on this page express your preference only. No live scheduling inventory or slot hold is connected. The hospital must confirm availability.

How do I choose the right specialist?

For IVF, fertility testing, IUI or ICSI enquiries, choose Fertility. For kidney conditions, dialysis-related advice or nephrology assessment, choose Kidney Care. You can also view the doctor's profiles before submitting.

How will I know my request was received?

The page will show a success message only if the connected hospital CRM confirms receipt. Otherwise, it will show an error and offer WhatsApp or a phone call as an alternative. A CRM receipt is still not a booked appointment.

What should I bring to the visit?

Bring previous prescriptions, relevant lab reports, medical records and a list of current medicines if available. Ask the hospital about preparation and consultation fees.

Can I request a same-day or urgent consultation?

Call the hospital to ask about current availability. For a medical emergency, seek urgent medical care rather than relying on an online form.
